The maximum plasma level of cinnarizine is reached 1-3 hours after ingestion. The half-life is 4 hours. Cinnarizine is completely metabolized by the body. Excretion of metabolites is carried out approximately 1/3 with urine and 2/3 with feces.
Plasma protein is approximately 91% bound by cinnarizine.
Pharmacodynamics
Neuro-Norm is a combination drug. The active components of the drug are piracetam, a cyclic derivative of γ-aminobutyric acid, and cinnarizine, a selective calcium channel antagonist.
Piracetam is a nootropic agent that acts on the brain, improving cognitive (cognitive) functions, such as learning ability, memory, attention, as well as mental performance. There are probably several mechanisms for the effect of the drug on the central nervous system: a change in the rate of propagation of excitation in the brain; strengthening of metabolic processes in nerve cells; improvement of microcirculation by influencing the rheological characteristics of the blood, without causing a vasodilating effect. Improves connections between the hemispheres of the brain and synaptic conduction in neocortical structures. After long-term use of the drug in patients with reduced brain function, there is an improvement in cognitive functions, an improvement in attention.
Cinnarizine inhibits contraction of vascular smooth muscle cells by blocking calcium channels. In addition to direct calcium antagonism, cinnarizine reduces the contractile action of vasoactive substances such as norepinephrine and serotonin by blocking the calcium channel receptors they control. Blockade of calcium entry into cells depends on the type of tissue, it results in an anti-vasoconstrictor effect without affecting blood pressure and heart rate. Cinnarizine can further improve insufficient microcirculation by increasing the elasticity of the erythrocyte membrane and reducing blood viscosity. Increases cellular resistance to hypoxia. Cinnarizine inhibits stimulation of the vestibular system, which results in suppression of nystagmus and other autonomic disorders. Cinnarizine prevents the occurrence of acute attacks of dizziness.

Dosage and administration
Adults
The drug is prescribed 1-2 capsules three times a day for 1-3 months, depending on the severity of the disease.
It is advisable not to miss a dose, especially if it is prescribed once. If this happens, the capsule should be taken immediately, but if it is time for the next dose, the missed dose should not be taken.
For symptoms of cerebrovascular origin: 1 capsule 3 times a day.
For symptoms of labyrinth disorders: 1 capsule 3 times a day.
Motion sickness: adults - 1 capsule half an hour before a walk, repeated every 8 hours. The last single dose should be taken no later than 17 hours to prevent sleep disturbance.

As part of the complex therapy of dyslexia, Neuro-Norm is used for children over the age of 8 years: 1-2 capsules 1-2 times a day, depending on the severity of the disease.
The course of treatment is 1-3 months. Do not use for more than 3 months. It is possible to conduct 2-3 courses per year.
The drug should be administered with caution to persons with kidney disease. In cases of mild to moderate renal insufficiency (especially if creatinine clearance is less than 60 ml / min), the therapeutic dose should be reduced or the intervals between doses should be increased.
Side effects
From the nervous system: confusion, nervousness, dizziness, hyperkinesia, ataxia, headache, insomnia, there may be a risk of worsening of the course and an increase in the frequency of epileptic attacks, vestibular disorders, imbalance, tremor, hypersomnia, lethargy, dyskinesia, parkinsonism, fatigue. Long-term use in elderly patients can lead to the development of extrapyramidal phenomena.
From the immune system: hypersensitivity, including anaphylaxis.
From the digestive tract: feeling of dry mouth, dyspepsia, abdominal pain, pain in the upper abdomen, stomach discomfort, diarrhea, obstructive jaundice, increased salivation, nausea, vomiting.
From the side vestibular system: vertigo.
From the side of the skin: angioedema, dermatitis, itching, rash, urticaria, photosensitivity, hyperhidrosis (excessive sweating), lichen-like keratosis, subacute cutaneous lupus erythematosus and lichen planus.
Mental disorders: irritability, drowsiness, depression, anxiety, confusion, hallucinations.
From the musculoskeletal system: muscle rigidity.
From the side blood systems and lymphatic systems: hemorrhagic disorders.
Others: asthenia, arterial hypertension, thrombophlebitis, hyperthermia, increased sexual activity. With a long course of treatment, an increase in body weight may be observed.

Drug Interactions
With the simultaneous use of alcohol and the use of drugs that depress the central nervous system, tricyclic antidepressants, the sedative effect may increase.
The drug potentiates the action of nootropic, antihypertensive and vasodilators. The use in conjunction with vasodilators enhances its effect, and the presence of cinnarizine reduces the activity of hypertensive drugs.
Neuro-Norm enhances the activity of thyroid hormones and may cause tremors and restlessness.
May enhance the effect of oral anticoagulants.
Diagnostic intervention: due to its antihistamine effect, Neuro-Norm may mask positive reactions regarding skin reactivity factors during a skin test, so its use should be discontinued 4 days before the test.
There was no interaction with clonazepam, phenytoin, phenobarbital, sodium valproate.
special instructions
The drug is prescribed with caution to patients with kidney and / or liver disease. In cases of mild or moderate renal insufficiency, it is recommended to reduce the therapeutic dose or increase the interval between applications.
Patients with impaired liver function should monitor the values of liver enzymes.
With long-term therapy in elderly patients, regular monitoring of renal function is recommended, if necessary, adjust the dose depending on the results of the study of creatinine clearance.
Penetrates through the filtering membranes of hemodialysis machines.
The use of the drug should be avoided in porphyria.
The drug may give a false positive result during doping control in athletes, as well as in the determination of radioactive iodine, due to the presence of iodine-containing dyes in the capsule shell.
Due to the fact that piracetam reduces platelet aggregation, it is necessary to prescribe the drug with caution in patients with impaired hemostasis, during major surgical operations or in patients with symptoms of severe bleeding.
Neuro-Norm can lead to irritation in the epigastric region; its use after meals can reduce the effects of gastric irritation.
You should refrain from the simultaneous use of alcohol or antidepressants, since the drug can cause drowsiness, especially at the very beginning of treatment (see section "Interaction with other drugs and other forms of interaction").
The preparation contains lactose. Therefore, patients with rare hereditary forms of galactose intolerance, lactase deficiency or glucose-galactose malabsorption syndrome should not use this drug.
Children
The drug is used in children over 8 years of age to treat dyslexia.
Pregnancy, lactation
Piracetam passes into breast milk, so if you need to use the drug, breastfeeding should be stopped.
Features of the influence of the drug on the ability to drive vehicles and especially dangerous mechanisms
Given the possibility of adverse reactions from the central nervous system, care should be taken while driving vehicles or working with other mechanisms.
Overdose
Symptoms: increased manifestations of side effects of the drug. In isolated cases of acute overdose, dyspeptic symptoms (diarrhea with blood, abdominal pain), a change in consciousness from drowsiness to stupor and coma, vomiting, extrapyramidal symptoms, arterial hypotension were observed. In children with an overdose, excitation reactions predominate - insomnia, anxiety, euphoria, irritability, tremor, rarely - nightmares, hallucinations, convulsions.
Treatment. There is no specific antidote. During the first hour after ingestion, it is necessary to wash the stomach. If warranted, activated charcoal may be given. Carry out symptomatic therapy. Perhaps the use of hemodialysis.
