The employee's personal card is the main form for personnel records of all employees in the enterprise. It is she who is the main form for military registration, while the inspector for personnel is responsible for the reliability of the data contained in it.

The header of the document is filled in first. Here you need to indicate the full name of the company and its code according to the OKPO classifier.

Next, enter the data in the tabular part. 1. General information

In paragraphs 9-10, the state of marriage is recorded, as well as data on the next of kin.

2. Information about military registration

This section is filled out only for men on the basis of a military ID or registration certificate.

In paragraph 2, the military rank of the employee is recorded. If he has not yet served, then here you need to indicate "subject to conscription."

Paragraph 3 reflects the composition, for example, “soldiers”, “medical”, etc.

Paragraph 4 indicates the digital code of the military specialty.

Paragraphs 6-7 reflect the name of the military registration and enlistment office to which the employee belongs and the state of registration. In the latter case, the entry is made in pencil.

The data specified in this section is certified by the employee of the personnel service and the employee himself.

3. Hiring and transfers to another job

In this section, data on the registration and movement of an employee is entered line by line in the table. Here you need to specify the date of issue of the order, the name of the structural unit and position, salary data, details of the order. The employee certifies each line with a personal signature.


4. Certification

It is filled in already in the course of work at the enterprise. This section contains data on the employee's certification. They must contain the date of the operation, the result of the work of the commission, the details of the protocol. 5. Professional development

To be completed as the employee completes the relevant courses. 7. Encouragement and awards

In this part of the document, you need to indicate information about rewarding the employee for high performance in work with diplomas, thanks, bonuses, etc. In the first column, you need to write down the type of award, and in the next - the name, number and date of the document, according to which the employee was awarded incentives.

8. Vacation

When an employee is given rest time, information from is recorded in this section. Here you need to enter information about all holidays, including without pay.

9. Social benefits

This section is filled in if the employee has any preferential working conditions. For example, disabled people of group 3 (working) should have a shorter working day. 10. Additional information

Personal cards of employees (Form T-2) are used in organizations for a complete and comprehensive accounting of personnel. Based on them, a file of personnel is formed. The unified form No. T-2 is used as a form, which reflects all the necessary information about the identity of the employee and his labor activity.

The form consists of 4 pages containing thematic blocks. The first and second pages contain general information about the employee and are filled out when he is hired; the third and fourth - are filled in the process of work.

1. What document is the personal card approved by?

2. Do all employers have to keep T-2 cards?

5. On the basis of what documents should T-2 be filled out when applying for a job?

  • an employment contract concluded with an employee;
  • an order (instruction) on hiring an employee;

7. How many years should personal cards be kept?

Documents for filling out the T-2 card of military registration

Rules for filling out a personal military registration card

When entering data into the T-2 form, a personnel officer must be guided by established standards. If the card contains incorrect encodings, it is considered damaged. You only need to enter certain codes. Their transcripts can be found in methodological recommendations or directly in the T-2 form.

Filling T-2 military registration

The personal card consists of four pages. All information in it is divided into 11 thematic blocks. Encoding information about the state of marriage:

All encodings must be with a personnel worker. The person being hired is not required to know them.

How to fill in T-2 "military registration"

This item must be filled out on the basis of a military ID or certificate. The employee provides these documents when applying for a job.

So, in the first paragraph, data is entered about the employee who is a reserve officer. The column "Citizens subject to conscription" is not filled. In the second paragraph "Military rank" indicate the position of the person liable for military service according to the entry in the military ID. This section must be signed by the employee after completion. He must also set the date of confirmation of the entered data.

Block 3 "Recruitment. Translations. Errors in filling out this section are not allowed. All data is entered on the basis of the order of the head to hire a new employee. The information in this block and in the work book should be similar.

Section 4 - "Certification". It contains the following data:

Information is entered into T2 on the basis of supporting documents provided by the employee.

The words and phrases “no”, “was not (a)”, “not available”, as well as dashes in the T2 form are not entered - such lines are left blank.

Grounds for entering information about employees

T2 personal card must be completed on the basis of the following documents:

An employment contract with an employee;

Organization orders: on hiring, transfer, leave, dismissal;

Passport (another document replacing it) of the employee;

Documents confirming the education received, courses completed, etc.;

TIN certificate;

Military ID.

Other information (for example, the degree of knowledge of foreign languages) is indicated only from the words of the employee. If necessary, the employer may request other supporting documents, in accordance with the specifics of the professional orientation of the organization, as well as the established internal regulations.

How to fill in the title?

The first page of the form contains information about the employee and the level of his professional qualifications. Filling out the T2 card (a sample of entering information is presented below) begins with the name of the employer's organization in the top line of the form. The name is indicated in full, in accordance with the statutory documents of the organization. In the coding table, the numerical designations of OKPO and OKUD are affixed. Filling in this section begins with the indication in the table "Employment contract" of the number and date of the employment contract concluded with the employee. Further, filling out the T2 card, a sample of which is presented below, is carried out in the following order:

In addition, OKIN contains specific codes for foreign languages, which must also be indicated in the coding table. Further, in the table below, you should indicate: the name, series and number of the document on education provided by the employee, the year of graduation, as well as the qualification obtained and the name of the profession in strict accordance with the document. The profession code according to OKSO is entered into the table. If an employee has provided two documents on education, for example, secondary vocational and higher education or first and second higher education, information about all available documents is entered in T2. Similarly, sections IV - VI include information on retraining, certification, advanced training received by the employee after employment.

Information on incomplete education should also be entered in T2, indicating the type of education available. For example, for an employee undergoing training at the time of employment, information about the education that was available before entering the current educational institution should be entered; on the basis of a certificate from an educational institution, indicate that the employee is currently studying, the name of the educational institution and the course of study. Similarly, information is entered if the employee was trained, but did not complete his studies, but expelled (or was expelled):

Information about the completed training, subject to the completion of three or more courses of an educational institution, is entered into T2 on the basis of a diploma (certificate) of incomplete higher education; at the same time, the designation “Incomplete higher education” and the course from which the employee was expelled are indicated;

7. Lines "7" indicate the full name of the main and, if any, additional position (profession) of the employee, which he occupies in the organization in accordance with the order for employment and staffing. Abbreviations or distortions of the name are not allowed. In the coding table of the section, the position code according to OKPDTR is indicated.

Work experience

It is entered in accordance with the previous work activity of the employee, the basis is the work book. If the employee is employed for the first time, zeros are entered in the first line “general”.

Information about the relatives of the employee: who must be indicated?

This section begins with an indication marital status employee at the time of employment. The words and phraseological units “married”, “in a civil marriage”, “cohabiting” and so on should be avoided: the line is filled with the wording determined by the all-Russian classifier indicating the OKIN code.
When changing marital status, amendments are made on the basis of a registry office certificate, indicating the details of the document.

Information about relatives directly residing with the employee, indicating the degree of their relationship, should also be entered in such a document as the employee's personal card. The T2 filling pattern looks like this:

All information is entered on the basis of orders for the organization, indicating the details of the order (date, number).

Important to remember! You should not indicate information about a change in the rate or salary carried out without changing the position of the employee in this section.

With all entries made in this section, the employee must be familiarized under the signature.

Holidays and rest days of the employee

This section is filled in as the employee is granted leave in accordance with labor legislation and internal regulations. In the section table, you must enter the details of the vacation order, the type of vacation, the number of calendar days. For annual leave, you should also indicate the period for which vacation days are given in columns No. 2, 3 of Table VIII (employee's personal card). The fill pattern looks like this:

Section X: What information can be provided?

"Additional Information" - a section provided for information that may be relevant to work, but is not indicated in other sections of the T2 form. Such information may include, for example, whether the employee has a driving experience and a driver’s license of a certain category, health restrictions (in accordance with a medical report), and so on.

How to correctly enter information about the dismissal

The last section is completed on the last day of the employee's work. You should indicate the details of the order, the reason for the dismissal of the employee and certify the entry made with the signature of a personnel employee. Filling out a personal card is carried out by a personnel officer. The employee for whom the personal card was filled out confirms the authenticity of the information contained in the card with a signature at the end of Section II, on the second page of the personal card.
To fill out a personal T-2 card, the following documents of an employee are required:

1) passport (or other identity document);
2) work book;
3) military ID;
4) a document on graduation from an educational institution:

Rules for filling out form No. T-2

1) The name of the organization is indicated in full. The abbreviated name is indicated if it is enshrined in the constituent documents.
At the same time, be careful in the correct spelling of the name of the organization.
For example, three different spellings of LLC "PARUS", "Parus", "PaRus".
2) The personnel number of the employee must include no more than six digits. It is assigned to each newly hired or employed person and does not change with any movement of an employee within the organization until his dismissal.
3) The taxpayer identification number and the number of the insurance certificate of the state pension insurance shall be indicated in accordance with the certificate of assignment of the TIN and the insurance certificate of the state pension insurance.
4) In the "Alphabet" column, the first letter of the employee's last name is indicated.

If the employee graduated from two secondary or two higher educational institutions in different specialties, both educational institutions are recorded.
14) Diploma qualifications are indicated in the following order:
- for those who graduated from a higher educational institution - "bachelor", "master", "specialist". For the qualification "bachelor" and "master" the direction is indicated, and for the qualification "specialist" - the specialty;

15) The profession (main and additional) is indicated in full on the basis of the staff list and the order (instruction) on employment.
It is coded in accordance with the All-Russian classifier of occupations of workers, positions of employees and wage categories ( OKPDTR). An additional profession is one that does not coincide with the main profession, but may be of interest to the organization.
16) Work experience (general, continuous, giving the right to a seniority bonus, as well as the right to other benefits established in the organization, etc.) is calculated on the basis of entries in the work book and (or) in other documents confirming the relevant length of service.
17) Married status is recorded and coded according to OKIN.

19) Passport data are indicated in strict accordance with the passport.
The address of the place of residence is recorded according to the rules for recording the place of birth.
It is necessary to fill in both columns - both the address of the place of residence according to the passport, and the actual address of the place of residence, as well as the telephone number of the employee or next of kin.

Completing the section on military registration

The main documents on the basis of which it is filled section II"Information on military registration" of the T-2 personal card are:
- a military ID (or a temporary certificate issued instead of a military ID), a certificate of a reserve serviceman for citizens in the reserve;
- certificate of a citizen subject to conscription for military service.

The personnel officer puts his signature, the name of the position and the transcript of the signature, thereby confirming that the entries were made by him personally on the basis of the documents submitted.

Entering information about hiring, certification, advanced training, incentives, vacations, social benefits, etc.

When entering information about hiring and transferring to another job, you must fill out all the columns of the table. Information must be entered without abbreviations.
AT section III“Employment and transfers to another job” information on hiring and transfers to another job is entered on the basis of an order (instruction) for employment (forms T-1 and T-1a) and an order (instruction) for transfer to another work (form T-5). When filling out this section, the employer must acquaint the employee with the entries made against signature.
When filling section IV"Certification" of a personal card, all columns and columns are filled. They indicate:
- date of certification;
- the decision of the commission, for example: “transfer to a position”, “send for advanced training”, etc.;

Data on advanced training are recorded on the basis of documents submitted by the employee.

AT Section VIII"Vacation" records all types of vacations provided to the employee during the period of work in the organization. The basis for making entries are orders for the provision of vacations.
When filling out the vacation table on the basis of the order, only the start date of the vacation is entered. The end date of the vacation is entered after the employee returns from vacation. This is due to the fact that in the event that an employee is called from a vacation or interrupted, the “End Date” column will contain the date the employee leaves for work, and not the planned end date of the vacation by order.
In the same table, leave without pay is necessarily noted. It is taken into account when calculating the length of service, which gives the right to the annual basic paid leave, since, according to Art. 121 of the Labor Code of the Russian Federation, it does not include the time for granting unpaid leave to an employee if their total duration exceeds 14 calendar days during the working year.
AT Section IX"Social benefits" should indicate the list of benefits and their grounds provided to the employee by federal laws, by-laws and local regulations of the organization ( employment contract, collective agreement, industry agreements).
AT section "Additional Information" for completeness of accounting, where necessary, indicate:
- information about training at full-time - part-time (evening), correspondence, external studies departments of institutions of higher and secondary vocational training (you should also put down the dates of admission to educational institution and its end)

After the employee is dismissed from the organization, entries are made in section XI of the reason for dismissal, which indicates the decoding of the reason for the dismissal of the employee in accordance with the exact wording given in the Labor Code of the Russian Federation, the date of dismissal, the number of the dismissal order.
When closing the T-2 personal card, the personnel officer puts his personal signature with its transcript and indication of the position. The employee himself signs in the same way. The signature of the employee in this case confirms his agreement with all the entries made in his personal card.

The procedure for making changes to information about an employee

When information about an employee is changed, the relevant data is entered into his personal card, which is certified by the signature of a specialist in the personnel department, and if changes are made to the sections “General information” and “Information about military registration”, then by the signature of the employee himself.
Changes are made in the same way as in the work book.
When changing the surname, the old one is crossed out and a new one is written in its place in accordance with the documents submitted to the personnel department.
Further, all subsequent changes in data about the employee (change of residence, etc.) are reflected in the personal card by the employee of the personnel department.
A complete replacement of a personal card is undesirable for the reason that in the future it will be difficult to restore all information about this employee.
As for changing passport data and marital status, the same scheme can be applied - with a strikethrough.
Encoding rules
When encoding information in the T-2 personal card, it is necessary to follow a number of filling rules defined by the Standard for automated personnel management systems:
1. If incorrect entries or information not related to this filling are made in the coding field, the T-2 form is considered damaged and must be rewritten. Therefore, it is recommended either to enter the exact codes, or not to enter them at all until the personal T-2 card is archived.
2. In the absence of records or in case of negative answers (“I don’t have”, “I don’t have”), the encoded part of this attribute remains empty.
3. When specifying dates, the name of the month is written in full, the year is written in four digits; for example: "February 14, 2006".
4. Dates are encoded in Arabic numerals in the following order: day-month-year; for example: "03.10.1998".
In organizations and enterprises that do not plan to introduce an automated control system for personnel activities, it is allowed not to encode details.
